EDIT:
Just realized that I didn't include the "Custom Shape" that I mentioned in the screenshot above. The shape I used for the buttons was simply a Rounded Rectangle with a radius of 2px.

It's a PSD file (which is used with the program Photoshop) that can be edited to meet your specifications and then sliced up and used as a MenuBar for a website.
